The federal agency charged with overseeing offshore drilling operations along the United States’ coasts has established a committee to investigate the fatal accident aboard a Fieldwood Energy Gulf of Mexico platform that tragically killed an offshore contractor on Saturday.
Fieldwood Energy Platform Investigation to Address Root Causes
According to a statement issued yesterday by the U.S. Bureau of Environmental Enforcement (BSEE), the expert panel of investigators, inspectors, and engineers will review information provided by operators, contract employees, witnesses, and subject matter expert interviews. They will also analyze any evidence from forensic testing.
Once the investigation concludes, the panel will issue a report detailing root causes and recommendations to prevent similar incidents in the future.
“Our sincerest condolences go out to the worker’s family and friends, said BSEE Gulf of Mexico Regional Director Lars Herbst. “We are assembling a panel of experts who will conduct a thorough investigation into this tragic incident. We are committed to a complete investigation to determine the cause of the incident and to identify approaches that will enhance the safety of future operations.”
Island Operator Contractor Tragically Killed While Working at Shut-In Well
The Island Operator contractor was tragically and fatally injured on May 15th while performing a non-emergency pressure casing test on a shut-in well at Fieldwood Energy’s Eugene Island 158 #14, a site located off the Louisiana coast about 50 miles south of Marsh Island. A second worker aboard the platform was not injured.
Fieldwood Energy is one of the largest oil and gas producers in the Gulf of Mexico, where it holds interests in approximately 500 offshore blocks comprised of over 1,000 wells and more than 500 operating platforms.
Fieldwood Energy’s last fatal incident occurred in November 2014, when an explosion erupted aboard the company’s Echo platform, another Gulf of Mexico facility located 12 miles from the mouth of the Mississippi River.
One offshore contractor tragically died in the blast, and three others were hurt, including one person who suffered serious injuries.
